Learning disability services

At Luna Care Services we provide care and support to adults who have learning disabilities and associated complex needs such as:

  • Communication difficulties
  • Autism
  • Epilepsy
  • Physical Disabilities

We pride ourselves on taking a person-centred approach which focuses on helping people with learning disabilities to reach their full potential and to maintain their independence.

We pride ourselves by getting to know each of our service users in order to understand their aspirations, goals and ambitions so that these aspects are reflected in their care and support plans.

We work closely with the person, and where appropriate the family members, friends, advocates and other professionals. As a result, a person centred outcome based plan that is tailored around the individuals choice, preference and needs will be produced.

We regularly review the support plans in place with the person, their family members and relevant healthcare professionals to make sure the right level of support is provided for the person to achieve their personal goals.

Luna Care Services works in partnership with multi-agency professionals to ensure the person receives all the support that is available to them which can include:

  • Learning Disability Community Team’s
  • Community Health Team’s
  • Community Organisations
  • Social Services
  • Housing Providers

We offer a flexible service in comparison to traditional services and are able to provide support in a number of settings such as:

  • In the person's own home
  • In the person's family home
  • In a supported living setting

The care and support provided is tailored to the individuals needs and the level of support can range from 30 minutes, up to 24-hours a day.

Our purpose is to assist people with learning disabilities to live as independently as possible in order to promote the person’s health, well-being and happiness.

Each person’s capabilities is assessed in order to identify what activities of daily living they are able to do for themselves, and where identified, additional support provided. This is to ensure they can remain active in a way that meets their individual need.

We provide support and guidance in order to empower the person to maintain their independence, promote responsibility and develop skills they need to live the life that they want.
